For your convenience, we accept cash, personal checks, and major credit cards including, VISA and MasterCard.  Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.  

 

 

Insurance

We are happy to bill most PPO insurance companies for you.  We are preferred providers for the following dental insurance companies: Blue Cross & Blue Shield of AZ,  Total Dental Administrators/Banner, Cigna,  Delta Dental Premier, Humana, MetLife, Premier Access, United Concordia-National Fee for Service, GEHA-Connection Dental Network Including: Assurant, some Aetna plans, Ameritas, United Healthcare (DBP),  Great West, Guardian and many more-- this list is always growing.  Please call our office for details 480-279-5233.
